Guest Health Survey

In order to protect members of the Southern Adventist University community, we are requiring campus visitors to wear face masks, have their temperature checked for fever at a temperature check station, and fill out the survey below. To gather the most relevant data, please fill out the health survey no more than 12 hours before arriving on campus. Please have each member of your party fill out a separate form. Thank you.

Health Survey

1. • Do you have a new onset cough (within the past week), a fever (100.4 or above), or have lost taste or smell?

2. • Have you experienced at least two of the following symptoms within the past 48 hours?

Chills
Headaches
Muscle pain or Fatigue
Nasal congestion
Sore throat
Nausea, vomiting, or diarrhea

3. • Have you been tested for COVID-19 and are waiting for your results?

If you tested as a requirement for travel (including return to campus) and have no exposure or symptoms, please answer "no".

4. • Are you or have you been in contact with any confirmed COVID-19 positive individuals within the past 14 days?

If you've previously answered "yes" and been cleared by the UHC for that exposure please answer "no".
If you are a healthcare worker or clinical student exposed while in full appropriate PPE, please answer "no".
